Leaving residential treatment is a milestone, not the finish line. Our structured addiction aftercare programme helps you carry your recovery into everyday life, lowering the risk of relapse during the months when it matters most. At Connection Mental Healthcare, your aftercare is planned from your very first day with us and shaped entirely around your needs.

Addiction aftercare is the planned, ongoing care that follows inpatient treatment. Where detox and clinical treatment address the immediate problem and any root causes, aftercare focuses on what comes next: staying well, rebuilding your routine and protecting the progress you have made. Continued care after rehabilitation is widely recognised as one of the most important factors in lasting recovery, which is why aftercare sits at the centre of how we work, rather than at the end of it.

It is far easier to step into life after treatment when the next steps are already in place. That is why we begin planning your aftercare at your addiction assessment, long before your residential stay ends. Together, we map out the type of follow-up care you need, so the move back to daily life feels considered rather than abrupt. By the time you complete treatment at Connection Mental Healthcare, you will have a clear, personal aftercare plan ready to follow.

Each aftercare plan continues the progress you began during your residential treatment.

Your aftercare is built around you and continues the treatment you began during your residential stay at Connection Mental Healthcare. Depending on your needs, it may combine several of the following.

Continued outpatient treatment

Your inpatient treatment continues on an outpatient basis, so the progress you have made does not stop when your residential stay ends.

A network of safe houses

Where a gradual transition is helpful, we can connect you with trusted safe houses so you have a settled place to return to.

Help with daily life

Picking up everyday life again often means sorting out practical matters. Through trusted partners, we can help you with things such as work, housing and finances. If additional mental health care is needed, our partner institutions are available to you.

Peer support groups

For those who find them valuable, we can help you get in touch with fellowships such as AA, CA or NA, so you stay connected to a wider recovery community.

FAQ

Frequently asked questions about aftercare

Why is aftercare important in recovery?

Aftercare bridges the gap between structured treatment and independent life. The weeks and months after rehabilitation carry the highest risk of relapse, and continued care during this period can make a meaningful difference to long-term recovery. We are here for you for as long as you need us.

How long does addiction aftercare last?

There is no fixed length. Because every aftercare plan is personal, it can last for as long as it remains useful to you. Many people stay involved for months or longer, gradually reducing contact as their confidence grows.

When does my aftercare plan start?

Your aftercare is part of your personal treatment plan from the start. Planning starts as early as your addiction assessment, so it is ready the moment you need it.
